Supreme Court Approved Standardized Forms

Effective July 1, 2013, The Ohio Supreme Court has approved and created standardized forms (28 total) concerning divorces, dissolutions, motions for change in the allocation of parental rights and responsibilities (custody and visitation) and child support, and parenting plans. Included below is the link to the forms on the Supreme Court website (both PDF and Word) that can be completed online or printed out for completion by hand:

  • Domestic Complaint Packet(PDF, 1MB)
    For new custody and/or support actions in Domestic Court when the parents are currently married to each other or in situations where the parties were previously married to each other. SEE ORC Section 2151.233.
  • Juvenile Custody Complaint Packet(PDF, 3MB)
    For new custody and/or support actions in Juvenile Court when the parents have never been married to each other or did not have any children born during the marriage. SEE ORC Section 2151.23 and 2151.233.
