RULE 39:
CONFIDENTIAL DISCLOSURE OF PERSONAL IDENTIFIERS
Pursuant to Rule 45(D) of the Rules of Superintendence for the Courts of Ohio, it is the responsibility of the filing party to omit or redact personal identifiers from case documents. Personal identifiers include social security numbers, except for the last four digits, financial account numbers, including but not limited to debit card, charge card and credit card numbers, and employer and employee identification numbers.
Personal identifiers should be omitted or redacted from all case documents submitted to the court or filed with the clerk, unless otherwise ordered by the court. Pursuant to Sup. R. 45(D)(3), all personal identifiers shall be provided to the court on the Confidential Disclosure of Personal Identifiers form promulgated by the Supreme Court as prescribed in and adopted by this rule.
The Confidential Disclosure of Personal Identifiers form shall be completed and filed by both parties in all actions for divorce, dissolution, legal separation and annulment. At a minimum, the party shall disclose their complete nine-digit social security number.
The Clerk of Courts shall seal all Confidential Disclosure of Personal Identifiers forms filed pursuant to this rule. Thereafter, the clerk may destroy the original form and shall maintain it in electronic form only. (Eff: February 6, 2010)
