RULE 20:

Support withholding orders; Instructions for service; Application for IV-D Services

A.    Withholding Orders; Seek Work Orders; Instructions for Service. 
  1. Withholding Orders or Seek Work Orders.  All withholding orders or seek work orders shall be submitted on the forms created by the Ohio Department of Jobs and Family Services

  2. Final Hearings. Complainants and/or movants for child support orders shall submit required withholding orders or seek work orders, and instructions for service at the time of the final hearing where an order for support may be entered, or as otherwise ordered by the court. 

  3. Temporary Orders for Support. The court shall order a specific party to submit withholding orders, seek work orders, and instructions for service when it makes a temporary order for support.

  4. Instructions for Service.  The instructions for service shall accompany any withholding order form.  These instructions for service shall include the parties to the case and either the employer or financial institution from which the withholding shall be made.    
 
B.    Support Orders to be Provided to the Child Support Enforcement Agency. Whenever a support obligation is issued or modified, the order or entry issuing or modifying such support obligation and the instructions for service which accompany the order or entry shall indicate that a copy shall be sent via the Clerk of Courts Office to the Franklin County Child Support Enforcement Agency.

C.    Required IV-D Application. Whenever a request is made for a child support order to be issued or modified, the parties shall complete and file with the Court, prior to the adjournment of the hearing, an Application for IV-D Services.
